~Do NOT guess at your name – look at your passport closely. (there is a fee for name changes after booking)

~ If you have a double name, list the names in the exact same fields as they appear on your passport.

~ this form does not allow an extra field for suffixes (Jr., etc.) so just list those after your last name on this form. (we’ll correct it on your booking)
